Responsibilities

Serve as a Leader of the Digi-Dibia healthcare team and convey the purpose and goals of

the service.

● Obtain vitals, history, physical examination and maintain medical records.

● Participate in the development, execution, and maintenance of treatment protocols.

● Follow up with health management care plans and goals with patients and other

healthcare providers.

● Provide information, counseling, and social support to patients. Coach patients on

effective management of their chronic health conditions and self-care

● Translate and interpret health information and laboratory results for patients in a

culturally appropriate pattern.

● Build and maintain positive working relationships with CHEW, Nurses, and other

members of the Digi-Dibia health teams.

● Collect data and report findings to the appropriate Digi-Dibia Health team member or

supervisor.

● Contact patients by phone, in writing, or in-person to ensure they have completed

required or recommended actions.

Qualifications

● Graduation from an accredited college of medicine in Nigeria

● Completion of the 1-year housemanship and National youth service corps

● Maintain an active medical license and be in good standing with the Medical and Dental

commission of Nigeria.

● Maintain and document activities, service plans, and clinical results in an effective

manner using the Digi-Dibia Electronic health record.

● Strong interpersonal and communication skills and the ability to work effectively with a

diverse community.

● Ability to plan implement and evaluate individual patient/client care plans.

● Skilled in the use of personal computers and related software applications including e-

mail.

● Continuously expand knowledge of community leaders, agencies, and resources related

to healthcare delivery.
